The Social Support Specialist is part of the Healthcare Management and Clinical Support team and is responsible for consulting on complex cases and developing care plans that can be implemented by care management staff to promote safety and well-being. The position will maintain an active caseload for purposes of consultation, training and direct care management support. In addition, therapists will assist with the completion of psychological assessments specific to IDD, such as ADOS-2 and psychosocial assessments.

Travel is primarily local during the business day, although some out-of-area and overnight travel may be expected.

• Collaborate on complex cases and develop a care plan with the Care Manager utilizing the Interdisciplinary Team and community support.
• Coordinate with the team to assist with safe discharge development and planning.
• Assess the individual’s situation, presenting problems and goals.
• Collaborate with other members of individual multidisciplinary healthcare teams to determine the level of need and identify appropriate community support.
• Develop relationships with community resources and share resources with the HCM and CM teams.
• Assist with referrals for assigned members.
• Advocate for individuals' access to resources and support and identify systemic barriers to advocacy in the administrative and executive systems.
• Complete occasional psychosocial assessments and screenings such as ADOS-2
• Recommend programs and interventions focused on improving the mental health of people with I/DD.
• Use analytics and data to identify and support individuals with chronic conditions and complex needs.
• Adhere to professional standards outlined by protocols, rules, and regulations.
• Maintain updated knowledge of relevant laws, regulations, and procedures.
• Meet all training requirements on time.

• Valid New York state LCSW or LMSW license required
• Master’s degree in Social Work (MSW)
• Valid New York State Driver’s License
• Ability to travel within New York State to perform occasional psychosocial assessments and screenings that cannot be completed virtually
• ADOS-2 certified
• Experience working with individuals with intellectual and developmental disabilities
• Preferred experience working with students in school systems
• Experience with electronic health record systems
• Excellent interpersonal communication skills
• Ability to maintain confidentiality and adhere to ethical standards
• Strong skills in organization, prioritization, and flexibility
